How Do I Make An Appointment? To schedule an appointment, please call (815) 397-8500. We will be happy to get you scheduled with our doctors, however, we do suggest that you call in as soon as possible as we do tend to fill our schedule quickly. Why Is This Appointment Different From Other Doctors I’ve Met With? Our doctors and staff are extremely thorough, you will never be known as a chart on the shelf. We take the time to know you personally in order to provide you with the best, and most specific care possible. What Do I Need To Bring To My First Appointment? You will need to bring the New Patient Paperwork, printed and completed, a photo id, and your insurance card if one is applicable. If you are unable to print the paperwork, please ask us to mail it when you call, pick it up from the office beforehand, or come to your appointment early. Our New Patient Paperwork is extremely important, and is used during your first meeting with the doctor.
